Application Forms for the 2011 - 2012 Grant Project

Grant application forms are available in three formats - Adobe FlashPaper, Microsoft Word, and Adobe PDF (see below).

Select from the links below the proper format that matches your software.  To download the documents right click the link and select "Open" or "Save".  This will allow you to download the file directly to your computer for review and printing and saving.  Clicking the link will open the document within your browser in the associated format, where it can be printed and/or saved also.

If you don't have MS Word on your computer save the Word version, then using WordPad (part of Windows), open the document.  You may have to reposition some of the pages using this method.

When mailing DO NOT MAIL AS CERTIFIED MAIL, as this slows down the application review process.  To confirm that we have received your application please call or email us.  Thanks!
